Hope Center

Hope Center is a 21-bed low barrier interim housing program located in Clearlake, CA.

At Hope Center, participants are asked to commit to three primary rules: One to choose health, two to live into their best self, and three to join in community. Upon entering Hope Center each participant receives a health screening by an on-site nurse. Two meals a day are prepared by an on-site chef. Case managers and housing navigators assist with barriers to housing. After leaving Hope Center for housing, individuals are paired with a Housing Sustainability Specialist who provides support for up to six months.

With the support of a case manager and nurse, participants create an individualized care plan to address their barriers. Some examples of services provided are:

  • Medical Care Management
  • Applying for SSDI
  • Employment Search
  • Resume Building
  • Document Recover-Identification Replacement
  • Enrolling in College
  • Budgeting and Credit Repair
  • Behavioral Health Services
  • Healthy Coping Skills
  • Reconnecting with Family or Social Support System

Hope Center opened on December 2, 2020 and was operated by Hope Rising until May 13, 2023 when Adventist Health assumed operations under our Community Health Programs.
