Medical Records for Beverly Hospital Patients

On September 7, 2023, the former site of Beverly Hospital and its adjacent medical office building began operating as Adventist Health White Memorial Montebello. Patients of Beverly Hospital, prior to September 7, 2023, may access their medical records in person at the Medical Records Department at Adventist Health White Memorial Montebello, 309 W. Beverly Blvd. Montebello, CA 90640, or by calling (323) 726-1222, ext. 4950 or 5020.
