Reporting Concerns

If you have a question or concern regarding the care you receive or the services rendered, please contact your nurse or physician. For non-medical issues, contact the appropriate department manager.

If the concern is not resolved to your satisfaction, you may file a formal complaint by calling the Adventist Health White Memorial Organizational Performance Department at (323) 268-5000, extension 78595. If your complaint is not handled to your satisfaction, you have the right to send a written complaint to:
