Malpractice Insurance and Claims History Verification for Adventist Health Providers

To request the verification of malpractice insurance and claims history information for an Adventist Health provider, please click the Request Verification button below or email your request to the Risk and Insurance Services department at AHRiskandInsurance@ah.org.

Be sure to include the provider’s full name and a release of information signed by the provider.

Request Verification

Adventist Health providers are covered through a combination of commercial insurance policies and Adventist Health’s self-insurance program. Upon receiving your request, we will gather and provide you with the necessary information.

Please note: If commercial insurance is involved, we will provide you with the contact information for the relevant insurance carrier(s).

If you’re unsure which malpractice insurance coverage applied during your time at Adventist Health, consider these common categories:

Adventist Health Employed Providers

  • Self-Insurance Program: Primarily covers Adventist Health employees.
    • Exceptions:
      • California Advanced Practice Providers Supervised by a Physicians Network Medical Group-employed Physician: Receive primary coverage through Physicians Network Medical Group’s commercial insurance policy with The Doctors Company.
      • Hawaii and Oregon Advanced Practice Providers and Physicians: Receive primary coverage through Adventist Health Medical Group’s commercial insurance policy with The Doctors Company.
      • Graduate Medical Education (GME) Resident Physicians: Receive primary and excess coverage through commercial insurance policies tailored to the risks associated with GME programs.

Non-Adventist Health Employed Providers

  • California Physicians: Due to California laws prohibiting hospitals from directly employing physicians, those working in our California hospitals are often employed by Physicians Network Medical Group and receive primary coverage through Physicians Network Medical Group’s commercial insurance with The Doctors Company.
  • Graduate Medical Education (GME) Program Faculty Physicians: Receive primary and excess coverage through commercial insurance policies tailored to cover their duties as faculty. As these programs are in California, these physicians may also work in other capacities in our hospitals and are also often employed by Physicians Network Medical Group, receiving coverage through Physicians Network Medical Group’s commercial insurance with The Doctors Company for their non-GME-related activities.
