Data breach by Signature Performance affecting Adventist Health Tulare patients

A data security incident was recently discovered by Signature Performance, an agency working on behalf of Adventist Health Tulare to collect payment for services. We have no reason to believe information has been misused due to this incident. At Adventist Health, we make every effort to protect our patients’ health information, the personal information of our associates, and our data systems. Signature Performance is in the process of informing all patients who might have been affected.

What happened:

Signature Performance detected suspicious activity on its network and immediately began an investigation. The investigation included the assistance of third-party specialists and determined that an unknown party accessed certain files on Signature Performance’s network. Signature Performance discovered that more than 70,000 patients were affected by this breach. Every case is different for each patient, and each patient will receive a letter from Signature Performance regarding what information is impacted.

Patient privacy is a top priority for Adventist Health. That is why we are partnering with Signature Performance to help inform over 70,000 patients who might be affected by this data breach.

Affected patients can call Signature Performance’s dedicated assistance line at (833) 566-7793 between the hours of 5:00 a.m. to 5:00 p.m. PST, Monday through Friday, excluding holidays. Patients may also write to Signature Performance at 10250 Regency Circle, Suite 500, Omaha, NE 68114. Please note that calls directly to Adventist Health Tulare regarding this incident will be re-directed to Signature Performance’s dedicated assistance line to ensure the fastest possible response to inquiries.

In response to this incident, Signature Performance implemented an array of additional security measures to further minimize the risk of a similar incident occurring in the future. In an abundance of caution, Signature Performance is providing individuals access to single bureau credit monitoring, credit report, and credit score services at no charge.
