The goal of the Community Outreach Medical Patient Advocacy and Social Solutions (COMPASS) Team is to support individuals with healthcare navigation and short-term care management. COMPASS is comprised of an interdisciplinary clinical team with medical, health, and social work services.

Our focus is to address social determinants of health that can negatively impact a person’s goals in their health and wellness.
