Our goal is for you to live well with diabetes
Poorly controlled diabetes may cause a variety of complications including nerve damage, trouble healing wounds and problems with eyesight and circulation.
At the Diabetes Resource Center, we personalize your treatment plan and help to coordinate other services or specialists you may need to see.
We will help you get control of the “ABCs” of diabetes:
- A — Hemoglobin A1c
- B — Blood pressure
- C — Cholesterol
Your primary care provider can refer you to the Diabetes Resource Center. Our nurse practitioner will complete an individualized assessment and work together with you to set goals for your success.
You should use the Diabetes Resource Center if:
- You have unanswered questions about your diabetes
- You can’t seem to get your blood glucose under control
- Your A1c is over 7.0 percent
- You want to tailor your treatment to meet your goals
- You are overwhelmed by the daily demands of living with diabetes
